A.)  CANDIDATE ACKNOWLEDGEMENT

       Before completing the HRTI SLP Application Form ensure that you as the candidate, has read the following:
       NOTE:  After reading each of the following documents, REMEMBER to close each page after reading to
       continue with the application process.

B.) PRE-REQUISITE NOTES

         Fees:  All applications must be submitted with the full annual fee for :
            Basic (3 months) full fee upfront
            Intermediate (6 months) full fee upfront
            Advance (10 month) 2x 50% payments per year (1st payment payable on date of enrolment and 2nd payment
            before the start of the 6th month)
          Photo:  You must include a digital colour face photograph of yourself.
          ID:   Please attach a copy of your Identity Book/Card or Social Security Document.
          Incomplete Application:   The Application will be considered incomplete without the complete application form,
            attached proof of qualifications, colour photo and the initial annual, or the first 5 months of tuition fees.
          Minimum age for enrolment:   18 years.
          Academic or Non-Academic enrolment:
            Basic (3 months) and Intermediate (6 Months) Short Learning Programmes is Non-Academic and therefore after
            completion of all Assignments will receive a Certificate of Completion.
            Advance (10 months) Short Learning Programme Students enrol as an Academic Student.
            PLEASE TAKE NOTE:
            Students who apply for the Basic (3 months) and Intermediate (6 months) Short Learning Programmes will not
            receive a Formal Certificate towards Credits but will only receive a Certificate of Completion.
            Students who apply for the Advance (10 months) Short Learning Programme will received a Formal Certificate
            towards Credits and will receive a Qualification Certificate. I acknowledge that I have read the additional

     H.) TERMS AND CONDITIONS (THE FOLLOWING ARE COMPULSORY FOR SUBMISSION)

               I agree to complete the full Short Learning Programme.
               I agree to pay the fees in full on time and in accordance with the agreement of HRTI to which I have enrolled
                 as a candidate.
               I agree to protect all material handed over to me as a candidate of HRTI in terms of the Copyright Laws and
                   only use it for my own study purposes.
               I agree that the responsibilities of the HSFS are not the responsibilities of HRTI and therefore cannot
                  held any party liable for each other’s negligence.
               I agree with this application to appoint HRTI as the Mentoring Institute for the duration of the studies here
                   here stipulated.
               I herewith select Therapon University, as the Higher Education Institution of choice to confer my Certificate
                   in any of the Advance Short Learning Programmes.
               I herewith acknowledge that in the case of the Basic and Intermediate Short Learning Programmes no
                   Qualifications will be conferred, but a HRTI Certificate of Completion will be issued at the completion
                  according to the HRTI set standards, rules, policies and procedures.
